PBM and Systemic Recovery, PTSD, and More!

In our last issue, I shared how PBM is moving from experimental to essential in neurology and pain care. This month, I’d like to go a step further.

From conversations with colleagues, patients, and in my recent discussion with Dr. Matthias Wojcik (podcasting coming soon!) One theme keeps coming back: light therapy is a shift in how we think about treatment.

Instead of only suppressing symptoms, we are learning how to restore function at the cellular level.


Beyond Symptom Relief: PBM and Systemic Recovery

In our upcoming podcast discussion, we explored how patients with complex, chronic conditions often face a dead end with conventional care. What we’re now seeing with PBM is:

  • Immune modulation – balancing overactive and suppressed immune systems
  • Neuroplasticity – supporting brain adaptation after trauma or disease
  • Energy restoration – improving mitochondrial function where fatigue has been resistant to treatment

 

Recent studies are reinforcing this systemic perspective:

  • A 2025 trial showed tPBM improved working memory in older adults by modulating resting-state brain networks.
  • In acute traumatic brain injury, PBM enhanced cerebral microcirculation and drainage, offering neuroprotection in the critical early stages.

Clinical takeaway: PBM is not a single-target therapy, it’s a systems-level intervention.


Spotlight: PBM and PTSD

A promising new direction is the use of transcranial photobiomodulation (tPBM) in patients with mild traumatic brain injury (mTBI) who also experience post-traumatic stress disorder (PTSD) symptoms.

A 2025 randomized, placebo-controlled trial found that patients receiving real tPBM showed:

  • Improved cognitive performance – significant gains in working memory and verbal learning
  • Better sleep quality – patients reported reduced sleep disturbances compared to placebo
  • Reduction in PTSD-related symptoms – fewer intrusive thoughts, improved emotional regulation, and overall reduction in clinical severity scores

 

Meanwhile, the placebo group did not experience these improvements.

Clinical takeaway: tPBM is showing measurable benefits in populations where PTSD, sleep disturbance, and cognitive impairment overlap. For clinicians, this opens a new pathway to support patients for whom conventional therapies may not be enough.
